Directive on the use of the Hologram Security Device (Meditag™) - Frequently Asked Questions (FAQ) on implementation requirements

Dear Registered Manufacturers and Importers,

Please ensure all information provided in this form is as accurate as possible.

The Ministry of Health (MOH) has made it part of their regulations that it is a requirement for all Registered Manufacturers and Importers to apply Meditag™ on all pharmaceutical products. Kindly refer to attached official circular by DCA (Ref: Bil (32)dlm BPFK/02/5/03 dated 6 August 2004).

Mediharta Sdn Bhd is committed to the smooth implementation of Meditag™ for all registered companies. It is extremely vital that adequate stock be available to fulfill your orders. Therefore, Mediharta seeks your co-operation to complete the Meditag™ Requirements Form in order for us to understand your needs and requirements.

The information that you provide will ensure sufficient stock will be reserved for your requirements. It is important for you to provide accurate forecast of your needs so that fulfillment of your order for Meditag™ may be timely and complete. To this end, your forecast for the 12 months beginning October 2004 is required to match your order accordingly.

Please complete this form and return the forecast to Mediharta latest by 15 September 2004, to ensure appropriate allocation of Meditag™ to your company. All forecasts are to be returned to Mediharta by e-mail, mail or fax to the address below.
